New Patient Registration
Streatham Hill Group Practice are accepting new patients. To register, please come in person to the practice for the registration form(s) or information.The doctors and staff at Streatham Hill Group Practice are proud to offer the highest standard of patient-centred healthcare. We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, minor surgery, childhood vaccinations and well-being check-ups. The doctors welcome new patients who live within our practice area. To register with this practice you will need to check that you live within the practice area. You will need to fill in a registration form and book for a New Patient Screening.
New patient official registration forms are available at the reception desk, registering online to be a patient is not available at this Practice you must come in person. You will need to bring a proof of ID with you as well as proof of address (e.g. a utility bill, lease agreement, council tax or bank statement with your name and address on it and dated within the last 3 months).
After completing your forms, it may take a few working days to formally register you with the practice. Forms that are printed from elsewhere will not be accepted. When you register with the practice it can take a few months before your medical records are received from your previous doctor. In view of this we will give you a questionnaire to complete, which will detail important information about your medical history.
PATIENT ONLINE ACCESS
If you already have an NHS number you are eligible to have patient online access, where you can book appointments, check results, request repeat medication and access your medical records. If you would like patient online access, please bring your photo ID to reception, once ID has been verified, reception can then give you the access codes to register your account.
REGISTERING YOUR BABY – Once your baby is born then it is important that you register the baby as soon as possible in order for their details to be entered on the system before they have their 6-week baby check and are due to start their immunisations at 8 weeks. Please bring the NHS number provided by hospital, when registering your baby.
