Improving Outcomes for Patients with Diabetes - workshop invitation

What is this meeting about?
The 7 practices who form Streatham Primary Care Network are working together to improve our care of our patients with Type 2 Diabetes. In particular, we are looking at how we ensure that each patient receives the required care each year. These are
- Blood pressure
- Foot check
- Blood tests to check cholesterol, sugar (HBA1C) and kidney function
- Urine test to check for micro-albuminuria (ACR)
- Record of smoking status
Currently we are struggling to get all our diabetic patients to have these checks done.
We need patients with Type 2 Diabetes to get involved in this project, so that any changes or improvements best meet your needs. As patients receiving this care, we think you will have good ideas about how things could be improved.
We have a target to engage with 300 patients with Type 2 Diabetes across our practices by the end of April 2020.
This meeting is to get ideas from you about how best to do this – this can be as meetings, workshops, surveys or other methods.
Why have I been invited?
We have invited all patients recorded in their medical records as having Type 2 Diabetes who are registered with the 7 practices in the network – these are Palace Road Surgery, Streatham Hill Group Practice, Valley Road Surgery, The Exchange Surgery, Prentis Medical Centre (Dr Masterton’s), Streatham Common Practice and The Vale Surgery.
Where and when is the meeting?
The meeting is in the ground floor meeting room of Gracefield Gardens Health and Social Care Centre, 2-8 Gracefield Gardens, London, SW16 2ST on Tuesday 11 February at 18.00-19.00.
